Probation Registration Form

Please read the following instructions before submitting your probation registration:

  1. Ensure that you have been uplifted and have cleared your SRC Fees
  2. Complete all required student information accurately.
  3. Select your programme and current semester carefully.
  4. Select only courses applicable to your programme and academic level.
  5. You may select courses from your current and previous semesters, where applicable.
  6. Ensure that your selected courses do not exceed the maximum of 12.4 credits or 41 notional credits (whichever is applicable)
  7. Check your course selections carefully before submitting the form.
  8. Submission of this form is a request for registration and is subject to Faculty verification and approval.
  9. Tick the "terms and conditions" box at the end, having read the terms and conditions carefully.

Terms and Conditions – Probation Course Registration

Please read the following terms and conditions carefully before submitting this form:

  1. Accuracy of information
    I confirm that all information provided in this form, including my student number, programme, semester and course selections, is accurate and complete.
  2. Responsibility for course selection
    I understand that I am responsible for selecting courses that are applicable to my programme and appropriate for my current level/semester of study.
  3. Maximum credit load
    I understand that, as a student on academic probation, I may register for a maximum of 12.4 credits (equivalent to 41 notional hours), as stipulated for probation registration.
  4. Course prerequisites
    I understand that selecting a course on this form does not necessarily mean that I have satisfied all prerequisites or other requirements for that course. My registration remains subject to verification by the Faculty.
  5. Verification by the Faculty
    The Faculty reserves the right to review my selected courses and amend, reject or require changes to my selections where they do not comply with the approved curriculum, prerequisites, academic regulations or the permitted credit load.
  6. Outstanding/failed courses
    I understand that I should select courses in accordance with my academic status and the requirements of my programme, including courses that I am required or permitted to repeat where applicable.
  7. Submission of the form
    I understand that I should submit the form only once and that any subsequent changes to my course selections should be communicated to the Faculty through the appropriate official channel.
  8. Declaration
    By submitting this form, I confirm that I have read, understood and accepted these terms and conditions and that I accept responsibility for the accuracy of the information and course selections submitted.
